ロングフォーム出生証明とは

出生証明書には詳しい版（ロングフォーム）と簡易版（ショートフォーム）があり、記載内容が大きく違います。ロングフォームは本人の氏名・生年月日・出生地・性別に加え、両親の氏名や詳細、出生登録の情報まで含まれます。ショートフォームは多くの場合、本人の基本情報のみで、両親の情報が載らないことが多いです。市民権の親子関係を示すには、ロングフォームが安全です。

IRCCがロングフォームを求める場面

IRCCの市民権証明（Proof of Citizenship）のチェックリスト（CIT 0014、2026年6月更新）は、すべての申請でロングフォームを必須としているわけではありません。必要かどうかは、申請の状況と、親子関係の証明が要るかどうかで決まります。

過去にカナダ市民権証明書の発行を受けている人や、カナダ国内で出生し市民権証明書を持っていない人には、ロングフォームを明示的に求めていない記載があります。一方、海外で出生し、カナダ人の親から系譜で市民権を主張する人で、市民権証明書をまだ持っていない場合は、両親の氏名が載った出生証明書（出生国の原本発行機関が発行したもの）が求められるため、実務上はロングフォームが安全です。ショートフォームでは親の氏名が記載されないことが多く、要件を満たしません。

また、IRCCは必要に応じて、該当する親、祖父母、さらに上の世代まで、親子関係とカナダ市民権の証明を求める場合があります。歴史的な特例として、1947年1月1日（ニューファンドランド・ラブラドールは1949年4月1日）より前にカナダで居住していた英国籍の人で、市民権証明書を持っていない人、またはその日前にカナダもしくはニューファンドランド・ラブラドールで英国籍の男性と結婚した一部の女性には、常にロングフォーム出生証明が求められます。

なお、CIT 0014では、婚姻証明書や死亡証明書について、ロングフォームとショートフォームの区別を記していない注記があります。

正しい書類の取り寄せ方

出生が登録された州・準州の戸籍事務に相当するバイタル統計局から始めるのが基本です。通常、最新の公式出生証明書や出生登録の写しは各州・準州のバイタル統計局で申請します。古い記録は、各管轄の公文書館に移管されている場合があり、年限は地域により異なります。

州・準州ごとに用語が異なります。例として、ノバスコシア、サスカチュワン、ニューブランズウィックは「ロングフォーム出生証明」という名称を使います。オンタリオは同等の書類を「親情報付き出生証明」と呼びます。親子関係が必要な場合は、単に「出生証明」を請求するのではなく、「両親の氏名が記載された公式の出生書類」を指定して依頼してください。どの書類を注文すべきかは発行機関が案内します。

多くの地域で、申請時に次の情報や書類の一部または全部が必要です。

• 本人の氏名
• 本人の生年月日と出生地
• 両親に関する情報
• 申請者の本人確認書類
• 記録を請求する権限の証明
• 手数料の支払い

実務上の注意点

親の氏名が載った出生書類が必要な場面では、名称にこだわらず、必ず親情報が記載される公式原本（または原本発行機関の正式な写し）を指定して取り寄せます。ショートフォームでは不備になる可能性が高いです。CIT 0014の最新版と、申請国・地域ごとの発行方式を事前に確認すると、遅延や却下の防止につながります。
